OUR CLIENT'S CHALLENGE

There are 4 VSD skids 400KVA with the configuration as follows: 400KVA Step Down Transformer – phase shifted 12 pulse operation PO:4505926933, 350-454KVA VSD, 12 pulse (but only has 1 breaker), PN:C544003049 and 400 KVA Step Up Transformer PO:45059269443.Found that there were no incoming cables that should be available from the VSD main breaker to the VSD converter. It should have 12 cables.

PRELIMINARY POST INCIDENT DISCOVERIES

4 units 400 KVA VSDs were reported in new condition since around 8 years purchased and no inspection was performed since they came.


IMMEDIATE ACTIONS

Reported to client's reps and the Supervisor, Conduct a meeting with client Supervisor and reps to review the finding, installing a type of hard cable-95mm2 ( 200 ampere) as the required cable type was not available on site. It should be a flexible cable AWG #4/0 (equivalent with 230 ampere). Due to smaller cable the VSD KVA is downgraded to 330KVA, Performing function and no-load test for all 4 VSDs witnessed by client reps.
